Project Tapestry Informed Consent

Welcome to Project Tapestry! Project Tapestry, a product and service of Otsuka Precision Health, Inc. (“OPH”), is a web-based digital resource intended to support women nearing or of menopause age as a data platform that combines self-reported structured health data with personal narratives to help women understand where they are in their menopause journey and guide informed conversations through personal insights.

By participating in Project Tapestry, you will provide personal information including limited demographic information (i.e. email address and location data), and health information (i.e. reproductive health information, “RHI”).

When you go to the website and create an account by providing your email address, you will be asked a series of questions that are based on an algorithm used to generate these questions and provide you with personal insights based on your responses. You have the option to share your personal experiences about your perimenopause and/or menopause journey.

This information will be shared with individuals who are engaged with work on Project Tapestry at OPH and third parties that OPH may contract with to deliver the related services. However, OPH has and applies reasonable and appropriate privacy, security, and technical controls to protect the confidentiality, integrity, and availability (“CIA”) of the information you provide to us. These controls include a secure web-based experience designed to protect the CIA of your personal information.

As a result, you will receive an Advocacy Toolkit, which will include personal insights related to your journey to share with your healthcare provider(s) and other individuals at your discretion. Project Tapestry is not intended to be a diagnostic tool or to provide and/or replace medical advice from your provider(s). If you are experiencing a medical crisis, please always contact your healthcare and medical providers or call 911.

OPH intends to use your personal information to de-identify and/or aggregate it to collect, store, share and/or sell to potential customers and third parties, and will have and apply reasonable and appropriate safeguards to protect your identity and to reduce and/or prevent the likelihood of re-identification to protect your privacy.

By using, participating, and interacting with Project Tapestry, you agree to the following:

Your personal information, including email address, location data, and RHI may be collected, used, shared and processed by OPH, its subsidiaries, affiliates, and third parties.
The legal basis for the processing of your personal information is your consent to processing.
OPH may use, share, or process your personal information to improve Project Tapestry product and services, and to develop new features and to make enhancements.
OPH may use, share, or process your personal information to communicate with you, including to share your Advocacy Toolkit and personal insights, for future research opportunities, and for marketing, including marketing communications of other Otsuka, OPH, and selected third party products and services.
OPH may use your personal information to de-identify and aggregate it, and to collect, store, and sell information this information, which constitute as secondary uses for commercial uses and purposes.

By using, participating, and interacting with Project Tapestry, you have been informed of the following:

You have the right to opt-out of any of these uses, and the processing of your personal information by OPH at any time, including communications as described above.
You have the right to exercise any of your personal privacy rights as outlined in the OPH and Project Tapestry Privacy Policies.
While we do our best to protect your personal information, your personal information could be compromised and there is a risk you may be identified as a participant of Project Tapestry if you use the Project Tapestry product, and your personal information is accessed by an unauthorized third party, i.e. someone who is not authorized to use, collect, or process your information.
OPH, its subsidiaries, and its affiliates may continue to have access to your personal information even after you stop using Project Tapestry unless you specifically opt-out of the use, and processing of your personal information, and/or exercise any of your privacy rights as outlined in OPH’s and Project Tapestry’s Privacy Policies.
Using Project Tapestry requires that you agree to its Terms of Service, and Privacy Policy, which can change at any time.
